2) Attach or scan completed form & email: info@nhp.training
3) Call to pay for your course(s) 510.553.1800 to pay.
APPOINTMENT REQUIRED
IN PERSON
1) Call first to make an appointment, then pick up Registration Forms at NHP Offices and complete in person
2) Pay via check, credit card or cash. For cash payments please bring exact change.
