REMOTELY
1) Print and complete the Admission Application & Enrollment Agreement.
2) Scan the completed forms and attach as .pdf to email to training.nhp@gmail.com, include a color copy of ID & social security card on one page.
3) Follow up email with a phone call to pay for your courses
4) Complete all state requirements: LiveScan, Negative COVID test results dated within 7 days of start date of training, TB test, Physical and Influenza shot during its season, and bring to them to the first day of class.
APPOINTMENT REQUIRED
IN PERSON
1) Pick up the Application Packet at NHP Offices.
2) Complete Admission Application and Enrollment agreement.
3) Pay for your courses and submit your identification at the Nightingale office.
4) Complete all state requirements: LiveScan, Negative COVID test results dated within 7 days of start date of training, TB test, Physical and Influenza shot during its season, and bring to them to the first day of class.
